An Outstanding Honor for Northern Metropolitan

Montefiore Health System recognizes that Northern Metropolitan delivers the highest quality of post-acute care to patients transitioning from hospitals.

The award is based on Montefiore’s comprehensive review of the following factors:

We are proud to have earned this outstanding honor and we look forward to working with Montefiore.

Northern Metropolitan earns prestigious Target: BP Gold + honor.

Northern Metropolitan has been recognized by the American Heart Association (AHA) and American Medical Association (AMA) for its commitment to helping improve blood pressure (BP) control rates among the patient populations it serves, earning Gold + level recognition as part of the 2022 Target: BP™ Recognition Program.

The Participation award recognizes practices that have committed to reducing the number of adult patients with uncontrolled BP.

Hypertension (high BP) is a leading risk factor for heart attacks, strokes, and preventable death in the U.S. In addition, there are 116 million U.S. adults living with high BP and less than half of those people have their BP under control – making both diagnosing and effectively managing hypertension critical.

“We are proud that Northern Metropolitan has received this great award from the American Heart Association for this year’s Target BP Recognition Program,” said Kenny Rozenberg, CEO of Centers Health Care. “The commitment to save lives as we continue the journey to fight hypertension demonstrates our dedication to our residents and their families.”

Target: BP is a national collaboration between the AHA and the AMA aimed at reducing the number of Americans who suffer from heart attacks and strokes each year by urging physician practices, health systems and patients to prioritize BP control. The initiative aims to help health care organizations improve BP control rates through use of the evidence-based AMA MAP BP™ quality improvement program, and recognizes organizations committed to improving blood pressure control.

Centers Health Care is committed to improving BP control rates throughout its facilities.

Nonprofit Skilled Nursing: Not for Profit, All for You

With lower hospitalization rates and more discharges home, not-for-profits like Northern Services are a positive choice for short-term rehab and long-term care. They
have a great nursing-staff-to-patient ratio and all profits after expenses are paid back into the facility—money spent on nursing staff, food, and other key quality-of-life
measures.

Nurses who work in nonprofit nursing homes are consistently satisfied with their jobs. A staff that’s less stressed out generally provides improved care. And content staff
members tend to remain longer at a facility. Low turnover rates mean that residents can bond with the nurses and other staff that provide their care.

Not only do nonprofit nursing homes have, on average, lower hospitalization rates, research also shows that they have fewer cases of pressure ulcers, and lower rates of
restraints. Patients appear to heal better in nonprofit nursing homes and are more likely to return to their own homes faster.

Protecting our patients, residents and staff from COVID-19

From the beginning of the coronavirus outbreak, our facility has followed the guidelines and recommendations of the Centers for Disease Control and Prevention (CDC) and the New York State Department of Health (NYSDOH). The safety of our residents, patients, staff and their families are our priority and we continue to take an abundance of precaution to protect them.

Our team is well-trained in the importance of proper handwashing and the use of alcohol-based sanitizer, and all are supplied with proper PPE. If they experience symptoms—such as fever or cough—they are instructed to stay home, and they must be cleared before returning to work.

Handwashing and hygiene posters are displayed throughout our facilities, and employee education and training continue based on the latest community and CDC guidelines. In addition, the COVID-19 vaccine is currently being administered throughout the Northern Services Group facilities to staff and residents alike.
